How should a plant structure failure codes in its CMMS?

A useful failure code system tells the next technician what happened and gives reliability engineers evidence for improving maintenance strategies. If every record says mechanical problem, repaired or other, the plant has captured activity but lost failure knowledge.

Record observations separately from conclusions

A symptom is what someone noticed, such as high vibration. A functional failure is the inability to meet the required duty. A failure mode is how the equipment failed, such as bearing seizure. A cause explains why it occurred, such as lubricant contamination. A corrective action states what was done. These fields should not be treated as interchangeable.

Allow unknown where evidence is insufficient. Requiring a technician to select a root cause during an emergency repair encourages guessing. The original observations should remain visible even when an investigation later identifies the cause.

Use equipment specific, controlled lists

Design the closeout around the technician

Test the choices with real work orders and actual technicians. Menus that require too many irrelevant clicks produce poor data. Supervisors should check whether selected codes agree with findings, not just whether a mandatory field was populated. Use examples to train people on the distinction between a symptom and a confirmed cause.

Illustrative example

A motor trips on overload. The recorded symptom is overload trip. Inspection finds a seized bearing on the driven machine. The failure mode is bearing seizure. Further examination identifies contaminated lubricant. The action is bearing replacement and cleaning of the lubrication system. Coding only motor trip would conceal the prevention opportunity.

Audit whether the data changes decisions

Sample corrective orders by asset class. Look for excessive other codes, contradictions and repeated failures that cannot be grouped. Measure whether the codes support bad actor analysis, PM optimization and spare parts decisions. Better coding is a means to better reliability decisions, not a goal by itself.
